Temp and CO2 appear to be fine, so that's not the problem. And clearly radiation levels are good.
Also, do you have some commercial zones?
This habitat is a bit on the small side, but not unreasonably so. You could space out your cross-paths a bit more, leaving more room for buildings, but I doubt that's the problem.
Yes I do have commercial zones. Though do the zones do better when right next to each other or can they be separated by open blocks? I think I will try starting with lower zoning to see what happens.
